Ankylos SynCone Concept-benefits and limits

Retrospective study of efficiency of the system based on 4 implants: 2007-2011

In nowadays world, when our life expectancy is constantly increasing, we are searching for solutions to improve and Comfort our everyday life. As the consequence, more and more elderly people become dental patients whose dreams to eat, speak and smile with confidence can become true thanks to dental implants. One of the most important benefits of dental implants is that they greatly improve the qulity of life. Dental implants allow to speak with confidence in a relaxed and natural tone. They can eliminate the numerous embarrassing inconveniences of removable partial and full dentures. Moreover, they restore chewing efficiency comparable to that of natural teeth. Since dental implants look, feel and function like natural teeth, they improve the cosmetic appearance of smile. All these benefits encourage people to look for a solution which is easy, simple, fast and relatively cheap. At the same time, it is very important for a patient to experience as little stress connected with the treatment as possible.

All these expectations seem to be catered for by Ankylos SynCone Concept. This study is to present the benefits and limits connected with the implementation of this solution.

Materials and methods

140 Ankylos implants (Densply Friadent) were placed between April 2007 and February 2011. All implants were inserted by one doctor in the private clinic. All drilling was made with the use of physiodispenser Frios (Densply Friadent). All patients signed an informed consent for the treatment. Patient selection included healthy patients with adequate bone quality and quantity in the area between mental foramen allowing for the placement of 4 implants with preferable implant length 11mm (8mm minimum) and minimum 3,5mm width with at least 35Nm torque at the moment of insertion. The bone quantity was assessed according to Lekholm and Zarb bone classification, whereas the bone quality was assessed according to Misch classification.

Results

140 Ankylos implants were placed and a number of prefabricated prosthetic parts of the Ankylos system were utilized between April 2007 and February 2011. One implant was lost due to a lack of osseointegration and movement (Table 1 and 2, Chart 1). Implant survival rate was 99,27%. Until today, no prefabricated elements used during treatment were found faulty (abutments or syncone caps).

Conclusion

This paper presents Ankylos SynCone Concept- immediate loading system based on 4 implants placed in the mandible between mental foramen. The study results support the hypothesis, that mandible overdentures placed immediately on implants inserted in the region between mental foramen are the most reliable procedure in implant-restoration rehabilitation of edentulous mandible. This procedure achieves high implant success rate, in this study 99,27%.

ñPatient sex and age had no influence on the spectacular results

ñResidual dentition condition or edentulism had no influence on results

ñImplant length or diameter proved to be of no importance for the success rate

ñAnkylos SynCone Concept system used in the study helps to maintain good hygiene and prevents plaque resulting in perfect peri-implant tissue condition, gingivae around abutments was health with no bleeding during periodontal probe.

ñStable alveolar ridge level around all implants was a consequence of good hygiene, proper overdenture and bacteria-resistant tight connection preventing micro movements on implant abutment interface.

ñAnkylos SynCone Concept implementation provides positive social and psychological reactions. Patients appreciate fewer invasive treatments and visits. Immediate loading solves cosmetic problems and ensures acceptable chewing comfort. The procedure has only one surgical phase, healing is transgingival, the area affected is anatomic where minimal augmentation is required. It does not influence the treatment time or prognosis. Overdenture is seated when anesthesia still works, reducing the pain. Overdenture acts also as protection and anti-swelling means.

ñOne incident of denture fracture happened to a patient with his own teeth in the mandible. A conclusion can be drawn that it is necessary to consider patient’s own teeth or implants when planning a dental treatment. In such cases a denture should be supported with a metal framework after six weeks from surgery to avoid fracturing.

ñThe exact fit between secondary structure( SynCone abutments) and tertiary structure (matrices and denture) prevents excessive horizontal force on the implants. The micro movements transferred to the implants by chewing, swallowing and talking are reduced to a minimum. To prevent excessive vertical forces as the cause of a functional overload. The patient is advised to eat only soft food for the first two weeks.

ñProfessional skills and knowledge have significant impact on the treatment.

Summary

Successful immediate loading of four implants in edentulous mandible with overdenture requires a few conditions to be fulfilled:

  • an implant with progressive thread, etched surface and tapered connection should be chosen to provide good primary stability, fast osseointegration and conical connection which prevents micro movements
  • 4 implants should be inserted in parallel and subcrestally in the mandible between mental foramen
  • immediately after the implant placement, it is necessary to splint them tightly with the use of special overdenture utilizing the system of prefabricated, precise elements
  • patients should be on a soft diet for the first two weeks to avoid excessive loading
  • a denture should not be taken off in the first two weeks of healing period, over the next four weeks it is recommended to take it off on a daily basis for hygienic purposes
  • balanced loading should be achieved via creating proper occlusal conditions.
